Need help identifying this bass!

Hi, i recently got handed this status by one of my relatives, and i have absolutely no idea what model this is. Its one of the maple necked ones, with active electronics and the hyperactive pickups. any help identifying this bass would be very much appreciated!
I'm pretty sure this is called the "Give it to Merton" model :D
